How would YOU "fix" D&D?

After reading a previous post and some 90 or so comments on the release of 4E, I offer this question:

How would you fix D&D? Or, to put it another way, what would your ideal 4E consist of?

I'm not looking for things that can be easily house ruled (get rid of AoOs) or easily swapped out with stuff that's already been published elsewhere (HP/vitality/damage save/injury). Many of the comments in the post mentioned a "fundamentally flawed" system. So what's flawed, and how would you fix it?
